I want to distribute code that I have written under an Open Source license .... how do I go about this?

There are many sample licenses at http://www.opensource.org/licenses/index.php and if you distribute your software under any of these licenses, you may say your software is OSI Certified Open Source Software. The Open Source Initiative (OSI) actually encourage the re-use of existing licenses, but if you do modify one or write your own, this page tells you where to go for official approval.

Do note - if you're going to distrubute under an open source license, you're encouraged to read and understand the license you're using, and also to consult with a solicitor, beforehand.

The Mozilla Public License 1.1http://www.opensource.org/licenses/mozilla1.1.php looks like a good starting point as it tells you (in section 6.3) how you may re-use it for your own code.
